Account Documents; add option to Client Name and Address to not combine Addressee information if the Last Names are the same

When printing some Account documents the Name shown above the address of Clients may be combined.

The rule is, if the Last Name of consecutive Clients is the same, the Name shown is combined, rather than showing each Client Addressee. This rule makes addresses for a Mr/ Mrs combination more familiar and traditional.

For example, the Name might show "Mr A and Mrs B Smith" instead of "Mr A Smith and Mrs B Smith".

However, this may not be desired, therefore a new setting has been added under Tools, Global Settings, Accounts group, General page "Combine Addressee information where Last Name is the same? ". The default is checked.
