New Account HTML Widget; Account Types do not respect Entity Account Type filters

The New Account HTML Widget was not respecting the Entity Account Type filters, all Account Types were available all of the time. If an invalid Account Type was selected an error was only shown at the end of the wizard - now the list of Account Types is updated as the Branch/ Entity changes.

Additionally, in order to better highlight that the Account Type is dependent on the Branch, Account Type is now entered after the Branch.
