Account Application Types; should only list "Account Application" Documents
Account Application Types were listing both "Account Application" documents and "Account" documents that support Account Applications on the Documents page. They should only list "Account Application" documents.
The list of Documents available to an Account Application include:
* Documents listed as available for the Account Application Type (now only includes Account Application type Documents).
* Documents listed as available and that support Account Applications for the Account Type - if an Account can be generated from the Account Application.
Other changes made include:
* Removing the "Fee" column from the Documents grid, as it is no longer applicable.
* Including Documents marked as available for an Account Application Type on the Document form, Usage grid, with the note "Available Document".
