Accounts; each client now has a "Privacy Consent" checkbox

Each Client on an Account now has a checkbox "Privacy Consent". This can be used to indicates that a signed privacy consent form has been obtained from the client.

In addition, on the Account Role there is a flag to decide whether the Privacy Consent option is applicable to the Role. If unchecked the Privacy Consent setting on the Client is not available.
