Account Details Report; update Standard Transactions section

Various changes have been made to the Account Details Report, Standard Transactions section. These include:

1. Remove "Limit Notes to a single line?" option.
2. Report columns re-laid out as per the Standard Transactions grid on the form.
3. "Next" column now displays no date if it has ceased.
4. New column, "Payment Details", added.
