Documents form; Fee and External Party pages are now disabled if not applicable

The Fee and External Party pages on the Documents form are now disabled if not applicable, i.e., the Document is not an Account type Document.

Previously, the controls on the page were disabled but by disabling the page, it appears greyed in the Page List which is more intuitive.
