HTML Widgets; widget.UI.DocumentManagerComboBox; Dynamically created controls error when "[New file...]" is selected and do not allow upload

Dynamically created widget.UI.DocumentManagerComboBox did not function correctly.

Selecting the "[New file...]" option appeared to do nothing but caused an error in the browser's developer console.
