Reporting; "Close Reason" can now be exported in Account List Reports

Reports which use the Account List as a base report can now include the Close Reason in the exported data.

When viewing a report not as an export, Accounts can also be grouped by Close Reason.

Note: These fields will only be populated for Declined and Unwanted Accounts where a reason has been entered.
