Client; added Date of Death field

A Date of Death field has been added to Clients.

To record a Date of Death make sure the Client Type is for a "Deceased Individual" and then the Date of Death field will appear (under Date of Birth).

Note, there is a Permission Key "Client.DateOfDeathUpdate" to prevents Users updating the Date of Death.
