Workflows; Some Item Outcome Icons displaying incorrectly

The following issues were identified and fixed relating to Workflow Item Outcome Icons:

• "Failed" Overlay was incorrect. It was displayed as a red dash and is now a red cross.
• "Invalid" Overlay was incorrect. It was displayed as a red cross and is now an orange exclamation.
• The following Overlays were missing entirely and have now been added:
• Refer
• Disabled
• Unknown
