Form Actions menu styling changes

The following changes have been made to the Form Actions menu, e.g., when clicking the Actions button in the bottom left of the Client form:

• Always shows as a list rather than grid of larger icons
• As more actions have been added, the "grid" view has become cumbersome and harder to read
• The "Records" heading is hidden, and the Record actions are now displayed horizontally along the top
• These actions are consistent on most record types and this layout saves a lot of vertical space
• Removed left padding from headings so the text aligns better with the underline
