Service Log form; Credit Enquiry tab; On small mobile devices, Credit Enquiry content did not use full form

On small mobile devices, the "Credit Enquiry" tab of the Service Log form did not use the full form. It only occupied about the top third.

NOTE: This will have been the case for other types of service reports, not just Credit Enquiries
