HTML Widgets; Account Withdrawal wizard; Schedule button is enabled and disabled incorrectly

The Schedule button on the Account Withdrawal (also Refund) wizard was enabled and disabled incorrectly.

It should only be enabled under the following conditions:

• Transaction Type is entered
• Date is entered
• Value is entered and is greater than zero
