Cost Centre List Report; may fail with an error like "Key 'NZGovernment' does not exist in collection"

The Cost Centre List Report may fail with an error like "An unexpected error occurred running Report 'Cost Centre List'. Key 'NZGovernment' does not exist in collection.".

This can occur where the database Country has been changed or the User Preference under Cost Centres "Show applicable Services for all Countries?" has been checked, non-local services defined, then unchecked again.

This means one or more services are still enabled in the Cost Centre, but no longer accessible.
