HTML Widgets; New Account wizard; Maturity Instruction not being saved when first creating Account

When first creating a Deposit Account via the New Account wizard HTML Widget, the Maturity Instruction and Reinvest to Account Type were not being saved.

Editing an existing "Quote" Account did not show this issue.
