Elements; new "Default Fee" flag to distinguish Default Fees from other Fees

A new "Default Fee" flag has been added to Elements to distinguish Default Fees from other Fees.

The database upgrade will update any Element defined as the Account Type's Default Fee Element (under the Accounting page) to a Default Fee.

Note, this has been added to cater for NZ High-Cost Consumer Credit Contracts.
