Bank Account Export Services; Lodgement Service should not allow GL Group Transaction together option

A Lodgement Bank Account Export Service allows the "Group Transactions together?" checkbox under "General Ledger interface options" section to be entered. Since this is not used for Lodgements, and will always be reset to checked, it has been disabled.

It also is no longer displayed on the Summary Page of the Bank Account form, Export Services page for Lodgements.
