Log Forms; Application Shortcuts including the "page" show the form but do not focus on the Subject control

If an Application Shortcut to open a Log form (e.g., the Account Log form) includes a "page" parameter, the form's standard behaviour is not respected.

In the case of the Log forms, if a record is specified in the Application Shortcut (e.g., an AccountId), the focus should go to the Subject control rather than the first control on the page.
