Summary Pages; template code not updated from 3.1.6.3

Summary Page template code was not updated for kb article #15512, meaning if the template code was pasted into a script the previous version was added.

Note, the latest script code is correctly run if the script was not overridden.

The following scripts have been updated:

"AccountApp_ApplicantEnquiriesAndDecisions" has been updated to version 1.03 on 24/09/2018.
"WorkflowItem_Supplemental" has been updated to version 1.04 on 24/09/2018.
