AccountTransaction table; review indexes

To improve performance of Direct Debit processing, the AccountTransaction table, "IX_AccountTransactionDirect" index, has been modified.

Previously this indexed the columns DirectDebitStatus, Date and AccountPk. This has been amended to DirectDebitStatus, ReversePk and Date. For SQL Server, the column "AccountPk" is an included column.
