Publish Documents wizard; added Monitor Category ranges

Monitor Category Ranges have been added to the Publish Documents wizard.

Note, these ranges apply to both Accounts and Account Applications, where used. For example, if you select a range for Monitor Category A, and this doesn't apply to Account Applications then all Account Applications will still be included. Therefore, you should check/ uncheck the classes of Logs on the Document Selection page to only include Accounts, if this is what you want.
