Task Manager; Account Applications grid not showing Monitor Category columns

The Account Applications grid on the Task Manager may not show Monitor Category columns. This was incorrectly using the Global Settings for Accounts instead of Account Applications.

In addition a new column "Created" has been added, which displays the date and time the Account Application was created.
