Account Applications; Data Entry forms not modal

Account Application data entry forms were not modal.

This means you could click on the "Edit via Data Entry Form" multiple times or leave the Data Entry Page Set form open and go back to the Application and change details.

Data Entry Page Set forms will now be modal to the Account Application form, meaning clicking back on the Account Application form simply takes you back to the Data Entry form.
