Account Applications; now record Closed User and Date/Time Closed on the database

Account Applications now record the User and Date/Time an Account Application was closed. Note, this is also available from reviewing "Audit" information.

The Account Application Summary Report now also includes this information as "Closed User". Previously this Report included a column "CloseByWhom", but this is not populated for Applications that convert to 'Open' Accounts.
