Account Quotation wizard; Account Details page fields re-arranged

The fields on the Account Quotation wizard, Account Details page, have been re-arranged.

In particular the Branch, Dealer, Broker and Manager are now entered first. This means the Dealer is entered before the Promotion, which is a more logical order.
