Workflows; 'Decision Card' and 'Bank Account Enquiry Review' items will not run if no 'Refer' outcome defined even if Decision Card does not support this

'Decision Card' and 'Bank Account Enquiry Review' items would not run if no 'Refer' outcome defined even if Decision Card does not support this.

A warning would appear and the User would be unable to action the Workflow Item.

The Workflow Item validation (including the validation in the Workflow Item wizard) now accounts for a Decision Card not allowing a 'Refer' type outcome.

NOTE: Automatic outcomes of 'Pass', 'Fail' and 'Refer' are still created in the Workflow Item wizard, regardless of the Decision Card being used.
