Credit Enquiry Reports tweaked to produce better PDF documents

The built in Credit Enquiry Report Scripts have been tweaked to produce better PDF documents.

The changes include:
Merging columns in the 'Defaults' tables to prevent the the right-hand side not fitting.
Increasing the overall font-size when producting a PDF (the HTML view remains unchanged).
Decreasing the page margins.
Hiding address map lookup icons.
