Workflows; Closing a Workflow from a Workflow Item now sets the Workflow Close Outcome

Closing a Workflow from a Workflow Item, e.g., a Question outcome, now sets the Workflow Close Outcome to the description of the selected outcome, including the outcome code.

The Workflow Close Outcome becomes either:
Outcome Code or
Outcome Description (Outcome Code)

Depending on whether the Outcome has as Description or not.

Actioning a Workflow Item of type Close Workflow or Cancel Workflow will set the Close Outcome to the description of the Workflow Item including the Item Code.
