Elements, Transaction Types and Departments; length of codes has been increased from 5 to 10 characters

The length of Element, Transaction Type and Department codes has been increased from 5 to 10 characters.

Reports have been modified to better size column widths depending on the largest code. This applies to the above as well and Account Type, Account and Client codes.
