Database documenter; new option to include Additional Index information for SQL Server databases

The Database documenter has a new option "Include Additional Index information?". This currently is only applicable for SQL Server databases.

When this option is checked, the following additional columns for indexes are included:

Index Type; the type of index, typically "Clustered Index" or "NonClustered Index".
Fragmentation; the index fragmentation percentage.

In addition, and currently only for SQL Server databases, an new column "Clustered" is always included. This notes whether or not the index is clustered.

Note, fetching additional index information may be slow.
