Revolving Credit Loans; tiered rate tables not working as expected

Revolving Credit Loans that use tiered rate tables are not working as expected. They are always picking the rate based upon a balance of $0.

In addition to correcting this issue, the Rate Table form has been reviewed and modified so that tiered items are no longer automatically sorted into numerical order. The user must enter them in numerical order. The final line is now always assumed to be the largest possible value, so is not saved to the database.

Note, this is a breaking change, but impacts Revolving Credit Loans only, which is currently in beta. After upgrading the rate for any tiered rates will need to be re-entered.
