Transaction List; many minor enhancements

A few minor enhancements have been made to the Account Transaction List Report.

A new option has been added to show the Notes column on a single line.

New columns:

Element Descirption
Transaction Type Description
Bank Transaction Id
Bank Export Id
Promise Value
Direct Debit Value (was a "Direct Debit" column but printed nothing)
Payment Method Description

In addition the default columns for the report have been updated.
