Pre Needs; adding a second Pre Need to an Arrangement may fail to save

When adding a second Pre Need to an Arrangement the save may fail with a duplicate key error.

This occurs when the global option to use the Arrangement code as the Pre Need code is checked. Since there is already a Pre Need with the Arrangement's code attempting to add another fails.
