HTML Widgets; Document Execute wizard; "Primary key does not exist in collection." exception when running an Email Document

When attempting to run an Account Email Document via the Reporting view (via the Document Execute wizard), the following exception may occur:

Primary key does not exist in collection.

The occurs under the following circumstances:

  1. Account, Email Documents
  2. User's Preferences do not define a default Client Manager
  3. User is not defined as a Manager under Admin, Users & Responsibilities
