HTML Widgets; Client Contact Method defaulting values incorrectly

The Client Contact Method HTML Widget was incorrectly defaulting the following fields to what was configured on the Contact Method, not what was previously saved against the Client Contact Method:

• HTML format supported (Email)
• Can Receive Faxes (Fax)
• Supports Voice calls (Phone)
• Supports SMS (Short Message Service) (Phone)
• Show 'Supports MMS (Multimedia Message Service)' option? (Phone)

This has been updated, and now the default values are only applied when a User selects a Contact Method Id from the Button List on the first page of the HTML Widget.

The following HTML Widget has been updated:
