HTML Widgets; Identification Type Edit; Improved display of "Show button to paste Name from Client?" and "Show button to paste Date of Birth from Client?" checkboxes

The "Show button to paste Name from Client?" and "Show button to paste Date of Birth from Client?" checkboxes in the Identification Type Edit HTML Widget were displaying incorrectly for Identification Types where the Special Type was not "None". Both checkboxes shared the same caption, which would make updating the settings difficult to follow as the context for each was only displaying if the Special Type was "None". Additionally, they were always indented, even if the Special Type was not "None", which is when they should be indented.

Both checkboxes have been updated to use the full caption (as per finPOWER Connect Desktop) and will also only be indented for Identification Types with a Special Type of "None".
