Account Close wizard; Refinance not showing initial details

The Account Close wizard, Refinance option, including the "Refinance" action from the Account form, has been updated to fix two issues.

• The "Refinance" page does not initially display the Refinance Summary.
• If the option is changed to "Existing Account" and back to "New Account" the "Next >" and "Refinance" buttons are not enabled correctly.
