Workflows; Outgoing Communication; Not respecting configured "Show Fee page" or Log Delegation options

The Outgoing Communication Log page of the Workflow Item wizard, when launched from the Workflow Types form, Items page, has options to "Show Fee page" and also a section on who the Log should be delegated to.

Neither of these options were being respected. The Fee page was always displayed and there was nowhere to view or enter Delegated User or Role information as there was on the Log and Incoming Communication pages.
