Account Application Sample Package; Rename of Account Application Type and HTML Widget and other miscellaneous enhancements

The Account Application Sample Package has had its ID's used for both the Account Application Type and provided HTML Widget to not use a dot. This solves the issue of having to specify a "Web Service Name" on the Web page of the HTML Widget.

When the Drivers Licence "Special Type" is "NZ Drivers Licence" the drivers licence version can now be entered in its own field. Previously, it relied on Users knowing to enter the version after the licence number separated by a dash (-).

Additionally, many code improvements have been made to the HTML Widget to modernise its implementation.
