Account Settlement Summary Document; updated and renamed Account Close Summary

The Account Settlement Summary document has been updated and renamed Account Close Summary.

This reflects the fact that in some countries using the term "Settlement" can be confusing as this may be considered when the Account is opened and disbursements are paid, i.e. settled.

The new company wizard now adds and uses this new Document.

The following standard Documents have been updated:

"Account_SettlementSummary.docx" has been removed.
"Account_SettlementSummary.xml" has been removed.
"Account_CloseSummary.docx" has been added.
"Account_CloseSummary.xml" has been added; version 1.05 dated 7/06/2018.

Note, the document code "ASQ" has been changed to "ACS".
