Debug Window "Action" added to several Admin forms

A new "Debug Window" action has added to the Action pane, under the Tools group, of the following Admin forms:

• Account Application Types
• Decision Cards
• Documents
• Page Sets
• Queues
• Report Templates
• Workflow Types

Note: the "Run" action on the Documents, Report Templates and Queues has been moved to Tools group above "Debug Window".
