Process Direct Debit Payments wizard; fixed several issues in the Printed List

Several issues have been fixed in the Report produced in the Process Direct Debit Payments wizard. These include:

1. "Account Name" column never included.

2. "Include" value may not be correct. Items that have a zero/ negative value may show as "included" - when they cannot be included.

3. Filter parameters at bottom of the Report shows in incorrect order.
