Summary Pages (Version 2); Custom form action links not rendering correctly which produced errors on Page Sets and External Party Organiser forms

Summary Table Actions added via the ISSummaryTable.Actions.AddFormCustomAction did not render correctly in HTML pages.

These actions should have produced a hyperlink with a "custom://" protocol but instead produced an "app://" protocol which would always be unrecognised as a form action.

The following actions were affected:

• Page Sets, Pages page, the "Edit Page Design" link in the Summary block did not work
• External Party Organiser, when an employee was selected in the right-hand grid and the "Add as User" link was clicked in the Employee Summary block, the link did not work

This issue was introduced in version 2.03.04 when long Application Shortcut support was added.
