Form Details report updated to show the Application Shortcut that was used to open the Form

The Form Details report which is displayed by right-clicking a form tab and doing Special, Form Details, has been updated to show details of the Application Shortcut that was used to open/ re-open the Form.

This is useful to see how an Application Shortcut can be formed, e.g., to produce an Application Shortcut to all an Account Application to be edited via a Page Set.

NOTE: The Form Details report has also been updated to use the Version 2 Summary Page styles.
