Creating a PDF from a Summary Page (version 2) HTML Document does not resolve styles correctly

When generating a PDF file from a version 2 Summary Page, e.g., a Decision Outcome Summary, the stylesheet is not resolved correctly meaning that fonts and other styles will be incorrect in the PDF file.

This was due to the "file://" protocol being used to reference the stylesheet. This has been updated to "file:///" (three forward slashes) to conform to the Windows standard.
