Documents; Ad-Hoc Account Documents; Departments and Insurers range filters not being applied correctly

Ad-Hoc Account Documents using Standard Account parameters allowed range entry of Departments and Insurers but did not apply these to the resulting SQL Where clause.

This was due to the business layer finBL.DocumentFunctions.GetAccountSqlWhereBuilder method not including Departments and Insurers and affected related methods such as finBL.DocumentFunctions.GetAccountSqlSelectQueryBuilder.
