Client Identification form; more tweaks with regards validation

The following updates have been made to Client Identification validation:

1. Mandatory fields now generate a warning.
2. Australian Drivers Licence; State now validated and is mandatory.
3. Australian Medicare; Colour now validated and is mandatory, Individual Reference Number is now validated separated to the Card Number, is mandatory and must be a digit 1 through 9.
