ISSummaryTableRows; new option for AddCaptionCodeDescriptionWithFormShow

A new version of ISSummaryTableRows.AddCaptionCodeDescriptionWithFormShow function has been added.

This option simply passes in the record's code and description. This is useful where the code and description are both known, eg where an Account object has already been loaded into memory.

Note, in addition, the existing "AddCaptionCodeDescription" and "AddCaptionCodeDescriptionWithFormShow" functions have been optimised. Depending on the type of record this may mean one less database query is used.
