Account Application Accept/ Decline wizard; removed declined time - as not used

The time declined has been removed from the Account Application Accept/ Decline wizard as it was not used.

The DateClosed field on the AccountApp table was always set to the current date, regardless of whether or not the user amended the date accepted or if declined/ unwanted the date declined. This has been corrected.
