Sample Script added to bulk add Workflows updated due to incorrect comments in code

The sample Script 'Templates\Sample Scripts\Script_WorkflowsBulkAddAccount.xml' has been updated to version 1.01.

The existing Script did not have the correct comments at the top of the Script code and the HTML page generated referred to the 'User Hierarchy' report which was incorrect.
